Home Care Services

Our Home Care Services consists of Personal Care Services, Home Aide Services, Companion/Sitter Services, and CAP/DA. Home health care is a wide range of health care services that can be given in your home for an illness or injury. Homecare is health care or supportive care provided by a professional caregiver in the individual home where the patient or client is living, as opposed to care provided in group accommodations like clinics or nursing home. Homecare is also known as domiciliary care, social care or in-home care

Peer Support Services

Peer Support Services (PSS) are an evidenced-based mental health model of care that provides community-based recovery services directly to a Medicaid-eligible adult beneficiary diagnosed with a mental health or substance use disorder. PSS provides structured, scheduled services that promote recovery, self-determination, self-advocacy, engagement in self-care and wellness and enhancement of community living skills of beneficiaries. PSS services are directly provided by Certified Peer Support Specialists (CPSS) who have self-identified as a person(s) in recovery from a mental health or substance use disorder.

Individual Suppport

Individual Support Services are “hands-on” individualized assistance with everyday activities that are required by a member with Severe and Persistent Mental Illness (SPMI) in order to live independently in the community. The services are intended to support adults ages eighteen (18) and older living in a private home, a licensed group home, an adult care home or a hospital that have a documented plan to transition to independent or shared housing. Additionally, this service may be used to help members maintain independent or shared housing.
